Driver Info

Get the name, address, phone number, driver's license number, state, expiration date, birthday. (Note if glasses or contacts are indicated, and if the driver appears to be wearing either.) If possible take a photo of License.

Insurance Info

Get the name of the insurance company, phone number, address, policy number, and expiration. If possible take a photo of insurance card.

Police Info

Officer's Name and Badge number along with Police Report number.

Give a Statement

Do not sign any document unless it's for the police or insurance agent. State only the facts, and limit your discussion to the police and your insurance agent.

Vehicles

Get the Year, Make, Model, Color, license plate number, state, expiration, and name, phone, and address of the registered owner. Take note of the damage to vehicles involved in an accident. If possible take a photo of registration.

Witness Info

Get the names and contact info for any witnesses and passengers. Ask if the will stay and make a statement for police. If they must leave the scene ask for their contact information.

Description

Take notes of your description of the accident and draw a diagram if needed. Look for possible hazards, etc.

Get Information

Be civil and polite to other parties involved, but do not admit guilt. Utilize photo or video taking ability if available.
